If you were not the driver of the vehicle stated when the vehicle was parked in the above manner, or you were the keeper of the vehicle stated when the vehicle was parked in the above manner but it was in the possession of another person (either with or without your consent), please provide any information that will lead to the true identification of the driver responsible by completing the form above within 28 days of this notice.
If you are a vehicle-hire firm and the vehicle was hired out at the time the parking took place, please let us know using the form and provide us with a copy of the hire agreement and a copy of a statement of liability signed by the hirer under that hire agreement.
